Living well privacy policy

We (Woking Borough Council) are collecting your personal data in connection with the services we provide in the Home Independence team which may include Careline, Independent Living Schemes (Brockhill and Hale End Court), Independent Support, Social Prescribing, Homelink, Handyperson and Community Meals. However, we cannot collect personal data concerning your health without your consent (if you have a registered carer who is able to act on your behalf, they can also provide consent).

The information below applies to all of the personal data we collect about you for the services. Please read this information and indicate whether you accept by ticking one of the boxes below. We will hold a record of your consent and you can withdraw your consent at any time by contacting us.

Use of your personal data

We will use your personal data to process any application for services, provide the services and to keep our records up to date. We will not use it for any other purposes unless the law allows us to do so.

In order to provide this service, your personal data might be shared with the services and organisations listed on the following page.

Personal data for Community Meals and the Handyperson services will be shared with Data Images Software Solutions Ltd who provide our computer system. They will also be informed of any requests you make regarding your data.

We will not share it with anyone else unless the law allows us to do so.

We will hold your personal data for no longer than 12 months after our last contact with you (Handyperson); or 7 years after your last contact (Careline, Social Prescribing, Independent Support, Community Meals, Independent Living Schemes (Brockhill and Hale End Court) and Homelink (for funding via the Housing Assistance Policy)). You can ask us to delete it sooner if you wish – this request can only be overridden if the law allows us to do so.

Who we may share your information with

Your information may be shared with:

  • Forestcare Alarm Centre (for Careline clients and council tenants)
  • Emergency Services
  • Your family, next-of-kin, Power of Attorney, friends, neighbours who you have nominated to be your emergency contacts or nominated key holder
  • Other relevant Woking Borough Council services you may be interested in or that are relevant to the service being offered, e.g. housing services
  • Other local authorities
  • Surrey County Council Adult or Childrens Social Care
  • NHS health professionals, Woking Hospice
  • Care agencies – the agency that is providing a service to you
  • Non Profitable Charities eg Woking Citizens Advise, Age Concern (Woking), Pyrford United Charity, Byfleet United Charity, Food Bank, Besum, Furniture Project etc (if applicable)
  • Housing Association, management company, private landlord (if applicable)
  • Parity Trust
  • Building Contractor(s)
  • Specialist architect or surveyor services if required
  • SCC Care Provider (For Residents of Brockhill and Hale End Court Extra Care Schemes)
