When completing your application, you must:

tell us about any criminal convictions (known as ‘disclosure of convictions’)
provide 2 identical passport photos
give a written statement signed by an approved professional to confirm it is you – this can be a solicitor, a person of standing in the community or anyone with a professional qualification
show your licensing qualification certificate – you must be listed on the accredited qualification providers: personal licence to sell alcohol – GOV.UK
give a current DBS certificate – if you do not have one you must request a basic DBS check – GOV.UK
show proof of your right to work in the UK, such as a scanned copy of your passport – check if a document allows someone to work in the UK – GOV.UK
pay the licence fee of £37
