Proof of your medical fitness

You must be medically fit and have a good level of vision to be a driver.

Download a medical examination form and ask doctor (GP) to complete it.

Make sure questions 2 to 4 in section 1 are fully completed. The medical report is valid for 6 months from the issue date.

The new driver initial application fee covers one submission of your medical examination form to our medical advisor. Any additional submissions required are chargeable.

It is important that if you feel you need to tell us about any further medical information, you submit it along with your medical examination form.
