We offer one-to-one appointments with our wellbeing adviser, who will talk to you about your concerns and factors affecting your health.

Once we have received your application, a member of our team will contact you to arrange an appointment.

Our adviser will then help to identify and organise suitable assistance. They can make use of our network of services, as well as local community and voluntary organisations. This is referred to as social prescribing.

Who can use this service

To use this service, you must be:

  • over 18 years old
  • registered with a Woking GP surgery
  • experiencing non-medical issues that require additional support

Our wellbeing adviser can help you to access support in:

  • remaining independent at home
  • opportunities for social interaction
  • practical help, such as cooking, transport, shopping
  • physical activity
  • benefits and budgeting advice
  • housing options advice
  • mental or emotional support
  • support around a disability
