Service steps after leaving hospital

Within two working days of being discharged from hospital, you will be telephoned by one our team for an initial wellbeing check. If required, your community meal deliveries will begin.

Within five working days of being discharged from hospital, you will receive a follow up visit at home by the community alarm team and, if appropriate, the handyperson service who will conduct a home assessment.

Within three weeks of being discharged from hospital, you will receive an initial visit from one of our wellbeing adviser, if appropriate.

At six weeks after your discharge date, you will receive a follow up phone call to review your progress and ensure that any ongoing services required have been arranged.
