How to make a change to a club premises certificate, including changing a name or address, surrender a certificate or request a duplicate certificate.

You must let us know of any change in circumstances. This includes:

  • a change of name, club rules and/or registered address of the club (fee applies)
  • if you wish to surrender a certificate
  • if you require a duplicate copy of a certificate following loss or theft
