Both the Met Office and UK Health Security Agency (UKHSA) have extended the red alerts for Surrey and the south east until 11pm on Friday 26 June.

- a plan of the premises
- a premises operating schedule
- a copy of the club rules
- a club premises declaration form
Pay the fee
You also need to pay the application fee.
