Leaving home or care, evicted, leaving hospital, leaving the armed forces, leaving prison, homeless due to relationship breakdown, homeless and elderly, homeless with a mental illness.

Leaving hospital and becoming homeless

Before you leave hospital, tell the nursing staff about your situation so they are aware that you will be homeless when you are discharged. They will ask the hospital discharge team to help if they can, and they may refer you to us.

We will assess your situation. If you fall into a priority need category, you may be offered temporary accommodation while your application is assessed.
