Going home after hospital discharge (Homesafe Plus)

The Homesafe Plus service supports patients and their families following a stay in hospital.

Support steps after hospital discharge

Before you leave hospital, a hospital member of staff will discuss and recommend the service options available to you. If appropriate, they will complete a referral form on your behalf and send it to us.

The following support will take place at certain times after you have been discharged from hospital.

Within 2 working days

You will be telephoned by one our team for an initial wellbeing check. If required, your community meal deliveries will begin.

Within 5 working days

You will receive a follow-up visit at home by the community alarm team. If appropriate, the handyperson service will conduct a home assessment.

Within 3 weeks

You will receive an initial visit from one of our wellbeing advisers if appropriate.

After 6 weeks

You will receive a follow-up phone call to review your progress and ensure that any ongoing services required have been arranged.
