The medical form for hackney carriage driver licence and private hire driver licence applicants is to be completed by the applicant's doctor for a fee set by the practice.

The form is then passed to a doctor on the Council's approved list of medical practitioners who will charge a further fee.

Applicants with recognised medical conditions such as diabetes, epilepsy and heart disease should be aware they may not be issued a driver's licence on medical grounds.

Once a licence holder, medicals are required every five years up to the age of 60, and every year thereafter.
