The SLAP is a body of professionals from organisations including Woking Borough Council, Social Services and the Primary Care Trust (Health Authority).
It takes applications from people with:
and makes recommendations on suitable housing that will take into consideration their support needs.
You should make an early application, especially if you may become homeless.
You can get an application form by contacting the Housing Needs team at the Council.
Your professional or support worker will need to complete the last section of the form. You should attach any up to date assessments or Care Plans to your application form.
The information on your application form is confidential and only used by the SLAP.
Once your application has been received, an assessment will be carried out if you are not receiving support from the Community Care Team (for learning and physical disabilities and sensory impairment) or the Community Mental Health Team (for mental health needs).
The Panel holds a regular meeting to consider new referrals and recommendations for suitable housing will be made at this meeting.
More information may be required a recommendation can be made so a decision may take some time. The type of accommodation recommended by the Panel may not always be available, for example there may not be any vacancies in a suitable rehabilitation hostel or staffed home.
The SNIP considers applications for Woking Borough Council sheltered and supported accommodation, for those aged 45 or over who require a lower level of support.
