Welcome to the Citizen Panel Personal Details Form

All residents over the age of 18 are eligible to apply to join the Citizens' Panel. Woking Borough Councillors, who make decisions based on results of questionnaires sent to the Citizens' Panel, are not allowed to join the Panel because of the possible conflict of interests.

Please complete each section of the form before submitting to the Council. Note that some fields are mandatory and these are marked with a red asterisk.
