To do this you will need to complete the form on
screen and when complete click on the Submit
button. This will display a new screen.
Please click the option to Download the printable
version of your completed form. This will open a PDF
version of your completed form in a new window which you will need
to print out.
You then need to sign the form on the Declaration
section and post it in. You will also need to send your
payment and proof of your identity with the form. For more
information visit the Data Protection page on our website.
Unfortunately we are unable to accept forms that do not have a
signature.
